Interactive tax forms
These PDF forms can be filled in on your computer. Save the form, complete the fields on screen, then print it. Use Tab to move between fields, or click a field with the mouse. Interactive fields are outlined in light grey so they are easier to find.
Our forms do not currently allow electronic signatures. Completed forms must be printed, signed, and mailed to the Occupational License Fee office.

Tips for using the interactive forms
- If a form is hard to read, increase the page magnification with the plus (+) control on the toolbar above the form.
- Press Tab to place the cursor in the first field, then Tab again to move through the rest of the form. You can also click directly into any field.
- When the form is complete, use the Print button to print copies for yourself and for the Tax Administrator.
- You can also save a completed form to your computer for your records. Give it a name you will recognize later.
